The Alliance for Health Equity is a collaboration between the Illinois Public Health Institute, more than 30 nonprofit and public hospitals, seven health departments, and over 100 community organizations across Chicago and Cook County. Action teams coordinate community health assessments, community health improvement plans, and community health needs assessments to inform policy and programs.
