Multi-State EHR-Based Network for Disease (MENDS) is a population-based chronic disease surveillance distributed data network. It aims to improve chronic disease surveillance and management through the use of electronic health records and health information systems. MENDS-on-FHIR demonstrates the ability to leverage the Observational Medical Outcomes Partnership (OMOP) common data model and Fast Healthcare Interoperability Resources (FHIR) standards for national chronic disease surveillance.
