Washington State has built a model for integrating Community Health Workers (CHWs) into care coordination across the state. By leveraging their regional network of Accountable Communities of Health (ACHs)—independent, nonprofit organizations designed to improve health system coordination — and through joint funding from both public health and health care authorities, Washington has embedded CHWs throughout an already streamlined health delivery system. In this way, CHWs can do their best work, connecting community members to local resources anywhere in the state through data sharing and cross-sector partnerships.
